Treatment Stacking to TPE: The Next Aesthetic Treatment Trends to Know
As demand for more advanced, preventative approaches to aging enhancement grows, so does the race to find the next aesthetic breakthrough. Which innovation will move from industry buzzword to mainstream game-changer?
The modern aesthetics industry, long plagued by fillers, lasers, Botox, and the nips and tucks of aging skin, is sitting on the precipice of its next era. As the age-old act of going under the knife retreats into the background, the next cohort of treatments is waiting in the wings. But this time, the focus goes even deeper than an injectable, with more consumers becoming hyperfixated on skin quality, collagen stimulation, inflammation, hormonal changes, metabolic health, or how to preserve their appearance over decades, not just years.
“We are moving away from simply correcting signs of aging and towards restoring the biology of younger, healthier skin,” says Dr. Maryam Zamani, founder of the eponymous Chelsea clinic. “The future lies in regenerative medicine, where we stimulate the body’s own repair mechanisms in addition to improving volumetric changes and skin imperfections.”
In 2023, plastic surgeons globally performed 34.9 million aesthetic procedures , according to the International Society of Aesthetic Plastic Surgery (ISAPS), up 3.4% year-on-year; 19.1 million of those procedures were non-surgical. This tracks: the non-invasive aesthetic treatment market is currently worth $44.6 billion, per Mordor Intelligence, and is predicted to reach $67.29 billion by 2033, fueled by demand for established treatments such as dermal fillers, laser resurfacing and chemical peels, alongside newer categories like skin boosters, ultrasound lifting, body contouring and collagen-stimulating technologies.
This new era of aesthetics is changing how much patients are willing to pay for treatments, too. “In many ways, it’s like buying a designer handbag. You know the level of craftsmanship and quality you’re getting compared with something mass-produced,” says plastic surgeon and aesthetics practitioner Dr. Ash Soni, who notes that patients are now paying for both the quality of the product and the expertise of the practitioner, while ensuring minimal downtime afterward. “The industry is becoming a form of luxury self-care, and with all of the innovation happening, it’s a very exciting space to be part of.”

South Korea has long been considered the beauty industry’s testing ground . While it was the first market to popularize now-seemingly omnipresent ingredients like salmon sperm, its next export isn’t a device, but a treatment philosophy: stacking.
Instead of relying on a singular device, stacking combines multiple treatments in a single appointment to target different concerns simultaneously. One laser may target pigmentation, another works on collagen production, a radiofrequency device works on skin elasticity, and a final device may work on overall skin texture. Then, to top it all off, a consumer might get a round of polynucleotides, the DNA-derived regenerative injectables used to target fine lines and skin quality.
South Korea is leaps and bounds ahead of other countries, where customers can undergo just about any aesthetic treatment.
The trend has been slower to take hold in the West, until now. Combining energy-based dermatological devices is not new, but many clinics have historically relied on manufacturer-approved protocols designed around individual device use. The challenge with newer stacking approaches is understanding the optimal combinations, sequencing, and safety considerations when multiple technologies are used in a single session. Unlike in South Korea, which has a stronger doctor-led aesthetic medicine culture, cosmetic laser and IPL (intense pulsed light) treatments can be performed by non-doctors in markets such as the UK and the US, creating much greater variation in training levels and clinical oversight.
“We develop our own technology in-house and have an in-depth understanding of how each machine works, meaning I can combine my pico, my aerolase and my Er:Yag laser in the same session,” says Dr. Saif Abbas Chatoo, co-founder of the Institute of Medical Physics, a London-based clinic that has its own technology development labs in San Marino. The difference, he says, lies in understanding how to use them beyond the basic instructions taught in standardized manufacturer training courses designed for those without medical or science degrees. “When you buy a laser from a company, it comes with a protocol sheet telling you exactly what to do. The issue is whether the clinician understands why they’re using those settings.”

The level of customization that stacking allows is becoming increasingly important as the market fills with more sophisticated technology. By combining complementary treatments, practitioners can create more tailored approaches to address a wider range of concerns within a single appointment. A pico laser, for example, is not a single device but an entire category, with different wavelengths, pulse durations, and clinical applications. The same is true of radiofrequency and other energy-based treatments.
For a clinic to stand out in the stacking space, the focus will be on the application and customization for a client’s skin type. “A lot of what happens in our industry is that practitioners see a new piece of technology, buy it, and then everyone performs the treatment exactly the same way,” says Dr. Chatoo. “There’s no real thought about why those settings are being used or whether they could be optimized.”
The next frontier of aesthetics is moving beneath the surface, into the biological systems that shape how we age. “The nervous system is the foundation that all the rest sits on. You cannot skincare your way out of a dysregulated nervous system, you cannot out-train it, out-supplement it, or out-treat it. If the body is locked in a state of stress, every other intervention is working against the current,” says Sarah Bradden, cosmetic acupuncturist and founder of The Bradden Method. “The most expensive facial, the most disciplined fitness regime, the most considered diet, all of it delivers a fraction of its potential if the system underneath is inflamed and braced.”
According to Dr. Barbara Sturm, chief product development officer of the eponymous skincare brand, this focus on the nervous system will manifest itself through a more integrated approach to beauty, centered around the body’s natural regenerative processes. “As a doctor and longevity expert, I’ve always emphasized the power of prevention over reaction,” she explains. “Supporting the skin before visible signs of aging appear allows our skincare and aesthetic treatments to work even harder for us.” (Dr. Sturm partnered with Bradden on an anti-inflammatory reset facial, available from this summer.)

Tools, including AI-powered skin analysis, biometric devices for heart rate and blood oxygen levels, and traditional Chinese medicine techniques like acupuncture, tongue analysis, and face mapping, are emerging to create a more personalized understanding of the factors influencing skin health. Clinics are also beginning to incorporate treatments designed to support stress regulation, from breathwork to acupuncture, sound therapy, somatic healing, to shift the body into a more restorative state.
One of the most closely watched developments in the longevity space is therapeutic plasma exchange (TPE), a decades-old medical procedure that is now being repositioned through the lens of preventative health. Traditionally, TPE has been used in hospitals to treat certain autoimmune, neurological, and blood-related conditions by removing a patient’s plasma, the liquid component of blood that carries proteins, antibodies, and other circulating factors, and replacing it with donor plasma or a substitute solution.
There is now scientific evidence that TPE could also support healthy aging by removing inflammatory molecules and other age-associated factors that accumulate in the bloodstream over time, creating a healthier biological environment. Reborne Clinic is among those to be introducing TPE to the UK’s longevity market through a partnership with Circulate Health. “Diagnostics have raced ahead. Many people can now measure aspects of their biological age, inflammation, sleep, or metabolic health, but the next question is, what do you do with that information?” says Reborne Clinic consulting CEO Faye Mythen. “TPE sits in the intervention layer of preventative health, and it has a medical foundation, which gives it a different starting point from many other longevity interventions.”

Still, TPE remains an emerging area of longevity medicine, rather than a proven anti-aging intervention. Plus, it is minimally invasive and carries potential risks such as infection and fluid imbalance. Whether TPE reaches the mainstream will ultimately depend on clinical evidence and regulation. “It could become mainstream, because it aligns with where healthcare is heading: earlier risk identification, more personalized diagnostics, and interventions focused on resilience before disease takes hold,” says Mythen. “But mainstream adoption will depend on trust, meaning evidence, standards, and safe clinical delivery.”
If Moxi and BBL defined the last wave of energy-based aesthetics, XERF (eXperience Exponential RF) is positioning itself as the tech-led treatment to watch.
Developed in South Korea, XERF is a monopolar radiofrequency device designed to lift and tighten the skin without surgery. What sets it apart from other traditional radiofrequency treatments is its dual-frequency technology, which is designed to target different depths of the skin, its 6.78 MHz frequency is intended to treat more superficial layers, while the lower 2 MHz frequency penetrates deeper into the tissue.
The technology surged in popularity across Korea before gaining FDA clearance in the US last year, where it has quickly gained attention from both practitioners and celebrities like Kim Kardashian. It then launched in the UK in July, after receiving its European CE marketing and regular approval weeks prior.
Unlike injectables, which work by relaxing muscles or restoring lost volume, XERF uses controlled radiofrequency energy to heat the deeper layers of the skin, including the SMAS (superficial musculoaponeurotic system), the same structural layer targeted during a surgical face lift. The immediate heat causes existing collagen fibers to contract for an instant lifting effect, while triggering the production of new collagen and elastin over the following months. The result is firmer, tighter skin.
One of the biggest differences from other tightening treatments on the market is patient comfort. “Monopolar radiofrequency treatment is virtually painless, so most patients don’t even need numbing cream,” says London-based aesthetic practitioner Dr. Michael Moore, noting that others like Thermage and Ultherapy are notoriously uncomfortable. After nearly five decades in practice, Dr. Dray’s Kensington clinic doesn’t introduce new devices lightly, but has recently invested in XERF due to its results. “The fact that we’re introducing this treatment says a lot about how highly we rate it. We’re extremely selective about which technologies we invest in, and XERF is our newest device in many years,” he says.
For years, the injectables conversation revolved around two components: Botox and filler. Then, regenerative treatments such as polynucleotides shifted the focus from replacing volume to improving skin quality and becoming one of aesthetics’ biggest success stories. Now, attention is turning to what could become the category’s next breakout innovation: injectable peptides.
Practitioners are currently exploring their ability to stimulate collagen and elastin production in areas such as the face, neck, and under-eyes to improve circulation and support tissue repair. Rather than simply filling or freezing, the goal is to influence the biological signals that tell skin how to regenerate.
Some of the newest peptide formulations are being developed to stimulate pre-adipocytes, the precursor cells that develop into healthy fat tissue. As the popularity of GLP-1 weight-loss medications continues to reshape aesthetic demand, Dr. Soni believes these treatments could eventually help address one of their most common side effects: the loss of youthful facial fullness.
But peptides remain a controversial and largely emerging category . While some iterations have established medical uses, many of the newer cosmetic and longevity-focused peptides circulating online lack robust clinical evidence and regulatory approval for human consumption. It’s not helped by the rise of unverified suppliers and the darker side of looksmaxxing-driven demand.
The internet’s latest biohacking craze promises to be a fix-all, but should brands be taking extra precautions before getting involved?
The evolution of injectables is also transforming the wider skin booster category. While many formulations have long combined hyaluronic acid with amino acids, newer generations contain higher concentrations of regenerative ingredients designed to improve skin quality while encouraging tissue repair. “The aim is not only to improve skin quality, but also to repair damaged tissue while delivering that healthy glow and freshness patients are looking for,” says Dr. Soni.
The industry’s most established injectable treatments are also evolving. Neuromodulators, the category that includes Botox, are becoming increasingly competitive, with new formulations differentiating themselves by offering faster onset, longer duration, and more tailored patient outcomes. “Relfydess is probably the biggest innovation we’ve seen in neuromodulators in the last 30 years,” confirms Dr. Soni. “One of its biggest advantages is how quickly it works: most patients start seeing results within one to three days, and the effects can last up to six months.”
For comparison, traditional Botox takes up to two weeks and lasts up to three months. “It is a slightly stronger toxin, though, so for patients who want a very subtle ‘baby Botox’ effect, it probably isn’t the ideal choice,” says Dr. Soni, who notes it is also more expensive than traditional Botox.
“For a long time, everyone’s focus was on the face. Then, attention shifted to the neck, because people started noticing a mismatch between a rejuvenated face and an aging neck. Now, that focus is moving even further down the body,” says Dr. Soni, who notes that hands, arms, knees, the abdomen and, in particular, the décolletage are emerging as some of the fastest-growing treatment areas.
One treatment benefiting from that shift is Sculptra Body. Best known as a collagen-stimulating injectable for the face, it is increasingly being used to improve skin quality across larger areas by encouraging the body’s own collagen production. “The décolletage is definitely an area I’ve been treating much more frequently,” says Dr. Soni. “The downside, of course, is cost. Because larger body areas require much more product, treatments are considerably more expensive than facial procedures. Patients have to be prepared to invest in them.”
